I have the same problem with QPC2 v4. It won't read QL floppies with either density which can be successfully read by QPC2 v3.12 and native QL+Gold Card. I'm using Windows 7. Will try XP later, I'll let you know.

I've installed QPC2 v4.02 on XP and that solved the problem! So don't throw away your old XP machine yet...
