Thinking about long file names on the QL, has anyone ever looked at QVFS - and whether it could be used as a basis for this in SMSQ/e and SD card drivers?

You could get long filenames fairly cheaply with normal QL filesystem just by chaining directory blocks. Just need to define a new "type" then hope old filesystem code ignores it!
