Of course a 256 and a 128 works. I notice Urs website had a download for a three part rom (which he joins in a video and tests on Qemulator. The download on his site is broken though. I also tested this with a PM ROM on qemulator, splitting it in 3, then joining the first two parts back. The first two I placed in the ROM and the third part went in the back ROM. I saw exactly the picture you are showing above.

The picture you put above of the QL nearly booting is what I'd expect to see if the end part of the ROM was 'missing' so if I take a standard QL and remove the second ROM ic , then this is roughly what I would see. I don't think the third part of the ROM is going to the correct address

Please expand on F2 doesn't work reliably? The last time I saw a 64K booting minerva, the 74LS245 had failed. The reason I asked about F2 is because I have also seen a state where pressing F2 half the time gives monitor mode or doesn't boot. The ZX8302 was at fault.

I guess if you know the spacing between the screw holes (centre to centre) and the diameter of the holes you could knock something up quite easily

I'm tempted to buy one with a ceramic cap to see what it does and if replacing it with a 2.7k resistor makes any difference

I've just seen something. On the front of your adaptor is a ceramic capacitor. on my card this looks like a resistor and is marked '2701'. I'll post some pics in a bit

if you can , test the adaptor in a pc, just to make sure it's not faulty. jp2 on the adaptor switches 5v to 3.3v. you currently have it on 5v
