Hello Andrew,

where can I find the Kindle file? I have no Kindle but just the Kindle for PC (Windows) Reader from Amazon. Can I read the book with Calibre?
Yes, Calibre can read the book.
Open Kindle for PC - From menu select: Tools - Options - Content and you will see where the book is saved on your hard drive
On my system the book was saved in subdir named B003ODIZL6_EBOK which included the book in azw format and 2 other small files (presumably license? or something used by Kindle app)

Thank you, I have found it and converted it to PDF. But there is no index, how did you got the index?

Hello Andrew,

Have converted it to PDF but I have no index. How did you get the index?
In Calibre - load book - select Convert Books - go to Table of Contents and check Force use of Auto-Generated Table of Contents.

In Adobe PDF reader load the book then select Bookmarks - the Table of Content is shown as bookmarks for each chapter

Thank you! It's better than nothing. But the printed version has a superb index, 50 (fifty!) pages. Well in Acrobat Reader you can use the search, but I think I will print it out and then such an excellent index would be nice.

Well, if you have time you can play with the TOC filters in Calibre and convice it to generate all the index.
But I had no time to investigate the tags and the correct syntax to do that

I checked the Kindle version - it doesn't have the index :(
It only has the TOC.

You can export the book as a DOCX word and create the Index by hand, but that is a lot of time-consuming work!
