I would exclude a PSU overload because my Ql is inserted in a pc case with double power supply, a standard pc PSU for floppies, Kilgus VGA 2.0, and an internal fan, and the original Ql PSU for the motherboard and peripherals.

Hard to guess from afar, but my main suspect would be the Multirom. Hermes and SuperHermes sit within the QL internal IO range and shouldn't collide with anything. SuperGold Card mainly as well, but should work. QIMSI NoGold uses the RO Port area exclusively, but MultiROM could easily play into ...

Yes, to run a QL OS with standard QIMSI, you always need a CPU on the QL side, not necessarily a SGC of course.
