Your Ser-USB in that video seems to be spending an awful lot of time sitting there not responding to a DIR command (3 minutes or so!). From what I can see on the video, it looks like the watchdog timer abandoned the I/O attempt as non-responsive. I think you forgot to issue a QM_GO command ...
